Piles
Piles are hemorrhoids that become inflamed and glandular masses. Hemorrhoids are masses, clumps, cushions of tissue in the anal canal – they are full of blood vessels, support tissue, muscle and elastic fibers sometimes dermis also attached to few hemorrhoids.
Types of piles : Types of piles on the basis of piles origin places : Piles can be of various types and may be internal (inside the anus) or external ones (outside the anus). Typically, internal piles occur from 2 to 4 cm above the opening of the anus. External piles (perianal hematoma) occur on the outside edge of the anus. The internal ones are much more common. Types of piles by presentation by clock : piles is of different types also on the presence of pile mass like 11 o’clock 5 o’clock and 7 o’clock positions
Piles, also known as ‘hemorrhoids’, are small, bluish swellings, comprising of enlarged blood vessels situated either just inside or just outside the anus commonly called internal piles and external piles. In case of bleeding, they are termed as bleeding piles.
What causes Piles:
There are several factors which increase your risk of developing piles. Some of the common factors that might lead to piles are:
- bowl habits
- Genetic tendency (due to weak rectal veins, and walls).
- Poor tone of muscle in the rectal region.
- poor digestion and Constipation (hard stool).
- Obesity and other health issues like diabetics and heart disease or chronic hyper tention patients who are taking antihypertensive drugs.
- Inactive or sedentary lifestyle.
- Chronic or prolonged cough.
- Pregnancy
- Excessive use of laxatives or enemas.
- Habitual weight lifting.
Because of all these factors, straining is needed to pass the small hard stools, which causes congestion in the network of blood vessels located inside the anal cushions. Gradually, these vessels enlarge and form piles. If the constipation further continues, they become large enough to be called second or third degree piles
Fistula
What is Fistula?
A fistula is an abnormal connection that forms between two organs or vessels that are lined with epithelial cells. Fistulas are generally the result of a disease condition but artificial fistulas may also be surgically created for therapy.
Examples of naturally occurring fistulas are those that form between the anus and an opening in the skin (anal fistula) or between the intestine and the vagina (enterovaginal fistula). An artificial fistula may be created, for example, between an artery and vein (arteriovenous fistula) when a person needs renal dialysis.
- Anal Rectal abscess is a collection of pus in the area of the anus or/and rectum.
- Fistula in ano is a tunnel like pipe, an infected track, abnormal connection between two different points in intestines, often deep within the body, to the skins surface, i.e. between rectum internally and peri anal skin externally.

Causes of Fistula
- Infection of an anal fissure.
- suppression of urges
- Sexually transmitted infection
- The following factors increase your risk for an ano rectal abscess:

- Diabetes
- Inflammatory bowel disease.
- Weakened immune system.
- Local trauma
Symptoms Of Fistula
- Severe pain associated with mild temperature and swelling in the anal region.
- Recurrent perianal abscess.

- Discharge of pus from the rectum
- Fatigue and general malaise
- Fever, night sweats, and chills
- Lump or nodule, swollen, red, tender at edge of anus

Fissure = parikartika
An anal fissure is a cut or tear occuring in the anus (the opening through which stool passes out of the body) that extends upwards into the anal canal. Fissures are a common condition of the anus and anal canal and are responsible for 6-15% of the visits to a colon and rectal (colorectal) surgeon. They affect men and women equally and both the young and the old. Fissures usually cause pain during bowel movements that often is severe. Anal fissure is the most common cause of rectal bleeding in infancy.
Anal fissures occur in the specialized tissue that lines the anus and anal canal, called anoderm. At a line just inside the anus–referred to as the anal verge or intersphincteric groove–the skin (dermis) of the inner buttocks changes to anoderm. Unlike skin, anoderm has no hairs, sweat glands, or sebaceous (oil) glands and contains a larger number of somatic sensory nerves that sense light touch and pain. (The abundance of nerves explains why anal fissures are so painful.) The hairless, gland-less, extremely sensitive anoderm continues for the entire length of the anal canal until it meets the demarcating line for the rectum, called the dentate line. (The rectum is the distal 15 cm of the colon that lies just above the anus and just below the sigmoid colon.)
People who develop severe pin pricking pain, burning and occasional bleeding after a bowel movement may have fissure. Generally, dietary and medical treatments can be prescribed in the office. For those people who do not respond, or for people who have had the symptoms for a long time, a minor surgical procedure can be performed which can cure the problem.
